• Short introduction 

• Desired date, time and location

• Duration or type of date (hourly, dinner date, etc.)

• Screening information, see below

Screening Process: 

Screening is appreciated but not required if you only have an inquiry, however screening is mandatory to confirm any bookings. Inquiries with screening information will always be prioritized. 
Any information you send me will remain private and will be deleted after meeting or anytime upon request.
 
Option 1: You may send me a link to your LinkedIn profile, which must have 100+ connections. I will send you a link to my profile (only created for verification purposes), please send me a connection request. I will not accept the request, this is only to make sure the profile is indeed yours. If your account doesn't have a recent photo, please send me a photo in the form, or by text/email.

Option 2: Attach to the form, or send by text or email a clear selfie of you holding your preferred photo ID card (passport, driver's license, provincial ID). If you don't have a Canadian ID, please use your passport.
 

Option 3: If you have seen other providers in the past, I accept references as a screening method if they have verified Leolist or Tryst accounts. Please note that I will still need your full name to confirm any date.
